Description

There is growing number of people with mental illness and people with accumulated, dual diagnoses. The project responds to the needs defined by the psychiatric care reform and expands and creates a system of follow-up services and care for mentally ill people. The project aims to build an integrated care program for mentally ill patients. Project activities will include the following: the development of social rehabilitation care program for patients, with emphasis on their individual needs; a new methodology for these patients, professional staff training, a purchase of new equipment and the establishment of a collaborative network of psychiatric institutes, social services etc. The outputs of the project will be therefore to create a comprehensive care program for patients (at least 30 patients per year go through the service, 45% is integrated into the society with secured housing), establish a supportive network (contacts between Aftercare centre and psychiatric institutes), implement a methodology for working with dual diagnosis patients, build a new computer training room for patients. The target group of the project are clients with a psychiatric diagnosis addiction and dual diagnosis.

Summary of project results

The aim of the project was to build an integrated care program for patients with dual diagnosis within Aftercare facilities of Společnost Podané ruce o.p.s. Implemented activities include the following: the development of social rehabilitation care program for patients with emphasis on their individual needs; a new methodology for these patients, professional staff training, a purchase of new equipment and establishment of a collaborative network of psychiatric institutes, social services etc. Project goal was fulfilled fully, an integrated care program is running and helping clients from target group. The outputs of the project were creation of comprehensive care program for patients, establishment of a supportive network (contacts between Aftercare centre and psychiatric institutes), implementation of a methodology for working with dual diagnosis patients, and putting into use a new computer training room for patients. Among outcomes of the project are: 1. Start and implementation of a comprehensive care program for our target group of patients with dual diagnosis 2. Number of patients involved - 31 3. Number of leaflets - 200 4. Purchase of equipment for a computer room and a kitchen for clients The target group of the project were clients of Společnost Podané ruce o.p.s’s social service so it was easy to get in contact with them. Moreover, a supportive network of doctors was created, along with aftercare centers and psychiatric institutes to reach the target group.
