Description

There are missing services of community care for mentally patients in the Czech Republic that would be focused on active seeking for patients and providing individual plans of treatment related to secondary (relapse prevention) and tertiary prevention (social integration). The project is focused on the de-institutialization of the care for people with mental health needs. The aim of the project is to enable a successful transition of mentally ill patients from institutional care to other forms of care and their successful and permanent integration into society. The output of the project will be the newly compiled comprehensive program of care for mentally ill patients. At least 25 people from the target group will benefit from the program. Second output is a strengthened and professionalized NGO from area of care for the mentally ill. The main target group are mentally ill people from the district of Beroun.

Summary of project results

The project focused on the deinstitutionalisation of psychiatric care. The subject of the project was the creation of a comprehensive program for the care of psychiatric ill persons in the district of Beroun, which Lomikámen has long been involved in. The aim of the sub-project was to enable the successful transition of patients from the institutional care to other forms of care and their successful and lasting integration into society through this comprehensive care program. The "community-focused" complementary activity was developed on the core of the project. The aim of this supplementary activity was to provide the representatives of the smaller municipalities of the Beroun District with basic information on mental illnesses, a network of social and health services for such ill persons (with emphasis on community care) and to invite them to cooperate with our organization. The core program of the project consisted of four activities devoted to the patient from his stay in the institutional setting, through the transition to outpatient care to the greatest possible integration into society. Activities were: work with a patient / client in an institutional setting (and his / her preparation for outpatient care); field work with the patient; work rehabilitation and education. As part of the complementary activity, the following activities were carried out: contact work with representatives of municipalities; personal meetings with representatives of municipalities and the beginning of cooperation and field work with selected clients in their natural environment.
