Description

The project addresses the need for community-based services provided at home for elderly/disabled people to avoid institutionalization. The project aims at providing 30 members of the target group with mobile care at home (communication, rehabilitation, assistance). First, the model of mobile services at home and telecare services for elderly/disabled people will be developed. Then a mobile team of a social worker, a medical expert and 3 social assistants will be set up and trained. Second, the needs for mobile services at home (feeding, shopping, rehabilitation) of the 30 participants will be assessed. Third, the mobile team will assist the 30 participants for 8 months. Fourth, a network of 15 NGOs and institutions will be set up to promote the model. Finally, the model will be presented to the stakeholders at an event. The project will benefit the elderly/disabled people, their families, relatives, 10 NGOs and 5 representatives of the public administration.

Summary of project results

The project addressed the need for community-based services provided at home for elderly/disabled people to avoid institutionalization. It provided a mobile service to 33 elderly people with disabilities in Sofia by mounting the communication service of setting off an alarm in case of risk and by providing direct support at home. The project goal was reached in full through the introduction and promotion of a model of integrated social services at home and direct support to the people in need. The integrated service can continue to be provided in the future making use of the capacity developed, i.e. a trained team, a working model and partners. The following project deliverables were developed: a model for integrated social services for the elderly (including with disabilities), a toolkit for assessment of the needs from mobile support at home, an analysis of the factors, needed for the development of integrated services, a Manual for decision-makers and long-term caregivers of elderly people. The following people took part in the testing and development of the model of integrated services: 33 users (elderly people, including with disabilities), 15 relatives and caregivers, 10 NGOs and 5 representatives of the public administration.
