The problem we have identified as the most pressing within the activities of our organization is the effectiveness of service provision related to the transfer of a pediatric patient with a serious cancer disease to a home environment. Currently the process is very disjointed - a member of the organisation receives the contact details of the client (the child''s parents) from the FN Team and contacts the client by telephone, searching for the contact details of the mobile hospice service in the region in non-standardised reports (excel spreadsheets, diary, telephone contact etc.).
