Description

The project is implemented in the region which is located in the most excluded area of the Czech Republic with high need of health and social services. The main aim of the project is to shift mental health care to more out-patient oriented care. The Project focuses on implementing a programme of systematic therapy for patients with chronic mental disorder. It aims to create a network or services which will link health and social care for patients. This network should be created by providers of therapeutic, rehabilitation services, terrain services, and subsequent care services, including protected accommodation and employment. The project should extend cooperation between network participants and thus improve the standard of care for the target group and create a complex multidisciplinary service network. Primarily the project will enable creation of a comprehensive programme for therapy and rehabilitation, so that patients will be well prepared for participation in social service network and their subsequent return to normal life.

Summary of project results

The project was implemented in the region which is located in the most excluded area of the Czech Republic with high need of health and social services. The main aim of the project was to shift mental health care to more out-patient oriented care. The project focused on implementing a programme of systematic therapy for patients with chronic mental disorder. It was established a network of services called “therapeutic community TAO” which linked health and social care for patients. This network was created by providers of therapeutic, rehabilitation services, terrain services, and subsequent care services, including protected accommodation and employment. Premises for the implementation of these new services were reconstructed and equipped. The project has extended cooperation between network participants and thus has improved the standard of care for people with chronic mental health problems and has created a complex multidisciplinary service network. Primarily the project enabled creation of a comprehensive programme for therapy and rehabilitation, so that patients are well prepared for participation in social service network and their subsequent return to normal life. One of key elements is work with patients based on their individual needs and problems. Methodology has been developed. 119 clients took part in this new programme. Out of this number, 106 were able to finish it. Direct support (common activities outside the hospital in communities) have used 378 clients and has helped to improve their social habits. Professional skills of 35 medical staff have been increased by trainings and attendance at 3 professional conferences. Implemented case management approach launched process of planning, searching, promoting and monitoring of services of health and social organisations in order to support clients return back to normal life and out-patient oriented care. These activities were completely missing in the region before the project implementation and have prepared condition for on-going psychiatric care reform in the Czech Republic. As a result of the project implementation, it was possible to decrease rate of re-hospitalisation by 5,3%.
