Description

The project will improve the quality of life and increase the efficiency of persons under care, by increasing the amount of nursing services and medical services on-site and in the field and also through facilitating rental of equipment for rehabilitation. The project is targeted to the elderly and chronically ill and dependent persons The following groups will benefit from the medical and rehabilitation services or training courses: 650 patients, 110 doctors, psychologists, physiotherapists and nurses who will improve their skills; 35 members of NGOs supporting the elderly, as well as social workers and guardians of elderly and dependent. The project will be implemented at the territory of Polkowicki and Glogowski districts. The problem of limited access to quality services tailored to the needs of dependent, the elderly and ill people has been diagnosed in a rural area, inhabited by 30% of the total population of the area covered by the project and in small and medium-sized cities – Polkowice, Głogów, Chocianów, Przemków. The project will introduce new benchmarks and standards that will significantly raise the level of care services for this group.

Summary of project results

The objective of the project no. 437/07/13 was the improvement of the health care for elderly, dependant and chronically ill people through the extension of scope of home, clinic and environmental care in Polkowice poviat. Activities implemented within the project were aimed at better adaptation of the health care to the need of the quickly growing population of chronically ill, dependent and elderly people. One of main activities of the project was a reconstruction of the clinic’s building to create the Gerontology Care Center. Such activities like: an installment of a computer network (LAN and unification of medical data system), adjusting the space for medical equipment rental, voice system installment were also implemented. In the project medical equipment was purchased. Within the project such services as rehabilitation, medical consultations, tele-consulting, psychological care, nursery visits were offered. Medical equipment was rented for rehabilitation purposes. The educational part of the project was also a very important issue. Trainings of medical personel and caregivers of dependent and chronically ill people also took place. Target group: The project was targeted to the elderly (60+) and the chronically ill and dependent persons from Polkowicki and Głogowski districts.
