Description

The problem we have identified as the most pressing within the activities of our organization is the effectiveness of service provision related to the transfer of a pediatric patient with a serious cancer disease to a home environment. Currently the process is very disjointed - a member of the organisation receives the contact details of the client (the child''s parents) from the FN Team and contacts the client by telephone, searching for the contact details of the mobile hospice service in the region in non-standardised reports (excel spreadsheets, diary, telephone contact etc.).

Summary of project results

Trained persons will benefit from being immediately connected with a worker who can access and open the program and offer options to families during the initial consultation. 

Not every family is able to make a decision right away, but there are situations when, unfortunately, they have to do it. And offering concrete and clear information at the very

 first contact  will greatly help families, members of the children''s support team and also hospital FN Motol.

Currently, it worked in such a way that offers in the region of hospice and supportive care were communicated only after consultation with the family, because it took time

to find specific links, to find out what experiences we have and don''t have in a specific region. We were only able to provide this information to the families on the second 

or third day, they had yet to make a decision and the team only then ensured the connection with a specific service. Therefore, the process took about a week.

Now after digitization thanks to the program - already at the initial consultation we can offer specific options and if the family''s condition allows and they are able to make 

a decision, we will be able to provide follow-up care and a dignified departure of the child in the home environment, (or another facility in the region) to be arranged during 

the initial consultation.
