Description

ACCESS project is based on local community realities related to persons in social risk: an insufficient offer of socio-medical services, a lack of access at medical services, an increased level of social dependence and aging. Main objective: to facilitate the ACCESS to appropriate socio-medical services for persons in social risk (elderly, disabled, adults) and decreaseing the effects of isolation,of socio-medical exclusion. Achievements:1. An innovative and new socio-medical center addressed to the persons in social risk. 2.Initiating an open local network and partnerships linking social and medical services, authorities and stakeholders.3. Local actions increasing the community awareness according isolation, marginalisation, risk situations and promoting volunteering and support. The project will benefit 1000 direct clients of socio-medical center as well as professionals and authorities involved in network. The community will become more motivated for inclusion, care and support.

Summary of project results

The need for the project emerged from the insufficient and inadequate socio-medical services and the limited access to certain medical services for vulnerable groups: persons in social risk, especially elderly at risk of marginalisation and isolation, and disabled persons (auditory, visual, motor, psychiatric). The main objective of the project was to develop a complex system of psycho-socio-medical services within the Socio-Medical Centre Dr. Teofil Mija (SMC). Secondary objectives were: developing a network of cooperation and support between social service providers and raising awareness and community involvement through volunteering. 19 partnerships were signed and 1138 people benefited of services. 12,829 medical and recovery interventions were provided in the medical centre, out of which: medical consultations – 4796, para-clinic interventions (EKG, echography) – 3829, rehabilitation procedures (including physiotherapy) - 4204. 50 participants attended the support groups, totalling 1070 participations. The socio-medical centre is operating as a new and innovative social service, providing a holistic approach (psychological, social and medical) for social cases, including promotion of a healthy life. Through the ACCESS Network, more openness for information, support, and transparency in decisions among local service providers was created. 7 network meetings were organised. 29 volunteers participated in the support groups and socio-cultural activities and 17 volunteers (students) at provided support in the socio-medical centre during their practice.
