Description

The main objective of the project is to promote health among older residents of Zgornja Gorenjska region. The population is aging. In Zgornja Gorenjska region the number of people over 55, which is the target group of the project, has already reached one-third of the population. The main cause for the occurrence of chronic diseases, which reduce the number of healthy years despite the increasing life expectancy, is unhealthy lifestyle. The project takes a holistic approach to solving this problem. The Programme for maintenance of good health for the elderly will be developed and implemented in the form of education, training sessions and workshops. The project will provide the introduction of changes in the lifestyle of elderly on mental, physical and spiritual level. Participation of older people in the project will be based on an individual approach. The Programme for maintenance of good health for the elderly will be carried out at seven different locations, thereby ensuring easier access for the elderly. The project includes all municipalities, associations of pensioners, elderly care nursing homes and adult education organizations working in the Zgornja Gorenjska region.

Summary of project results

The project seeks to respond to ageing-related challenges individuals and society face on a daily basis. The number of people over 55 (the project’s target group) has already reached one-third of the population in the Upper Carniola region. The leading cause of growing occurrence of chronic diseases, which reduce the number of healthy years despite increased life expectancy, is an unhealthy lifestyle. The main objective of the project was to promote health among older residents of Upper Carniola region; the objective was fully achieved. The project adopted a holistic approach and included the development and implementation of the Programme for maintenance of good health in the elderly in the form of education, training sessions and workshops. The project was tailored to suit the needs of each participant by considering their specific needs and limitations. 450 individuals participated in the Programme for maintenance of good health of the elderly and gained knowledge of and skills for leading a healthy lifestyle by exploring the mental, physical and spiritual aspects of it. The outputs of the projects were the following: a network of organisations that delivered effective expert health promotion initiatives targeting elderly population; the Programme for maintenance of good health of the elderly divided into six modules; 211 individual plans for maintaining health of the elderly which contained the six modules of the Programme and at the same time responded to the situation and met the specific needs of each individual participant; 810 hours of Programme for maintenance of good health of the elderly implemented for 450 participants in all seven municipalities; sustainable health promotion strategy for the elderly delivered in 7 municipalities of the Upper Carniola region with specific information for each municipality. The final evaluation showed that the project activities significantly raised health awareness, particularly in terms of the link between lifestyle and health-related problems. Besides, a health promotion strategy deriving from the project’s results and findings provides positive long-term impact on the elderly.
