Description

Currently there is no single comprehensive, coordinated and adapted for youth health needs health care delivery model in Lithuania. The goal of the project is to create and implement youth-friendly health services (YFHS) model in Lithuania whose main function - through service delivery protocols (pathways) to combine into a single network existing health care services and improve cooperation among these to provide effective youth-friendly health services and ensure the reduction of inequalities in health. YFHS model include: • Establishment of Coordinating Centre. • Establishment of National internet portal in which youth can obtain reliable, systematic, easily accessible information. • Establishment of YFHS institutional and functional network based on principle of case management. • Elaboration of methodological guidance for YFHS model implementation and provision of youth-friendly health services. The project consists of a development, validation, adaptation of YFHS model, pilot implementation of the model in Rokiškis municipality and consulting municipalities implementing model in a later phase.

Summary of project results

The project „Development of a Model for the Provision of Youth Friendly Health Care Services“ consisted of a development, validation, adaptation of YFHCS model, pilot implementation of the model in Rokiškis municipality and consulting municipalities implementing model in a later phase. A model of provision of youth-friendly health care services has been developed, which includes not only treatment services, but also prevention, risk assessment and monitoring. The model is versatile and in that it can be installed in any municipality, regardless of the existing infrastructure of service providers, because the proposed network of service providers builds on the need for services based on functions that are universal both in healing and maintaining health regardless of the situation in any area. The model suggests not creating new structures or institutions, but based on existing infrastructures existing in the municipalities and recommended health problem solving algorithms developed during the project, better organization of provision of services in the municipality - improving coordination and cooperation between different institutions and sectors, reducing duplication of services and reducing risk health assessments and / Services for the young person. The main elements and tools of the YFHCS model are to create better coordination and cooperation opportunities for service delivery: Coordination Center / Coordination function: The YFHCS Coordination Center at the Municipal Public Health Bureau or other unit where the coordinator is a public health or other specialist. The coordinating center carries out coordination and case management functions. Central Coordinating Center - Center for Health Education and Prevention. Algorithms are a tool for regulating network activity (algorithms for providing services to young people according to specific health needs). Preparation of 6 algorithms descriptions. The Youth Health Web Portal is an information gathering and communication tool (reliable, systematic, accessible form information and health communication platform for young people and service providers). The health-care portal is functioning. Training for professionals working on youth health issues (contact technique, motivational interview, case management and interdisciplinary collaboration, work with the Youth Health web portal). Trainings for specialists from municipalities implementing the JPSPP model were organized.
