Description

Psychiatric care in the Czech Republic has not undergone any major systemic changes since the beginning of 1990s. The project is thus a reaction to the newly established psychiatric care reform. The main objective of this project is to extend the network of services and to link social services in the district of Cheb with the healthcare system by establishing a new psychiatric clinic for severe mentally ill. This ambulatory service will be associated with the services of social rehabilitation and supported independent housing. The secondary objective of the project is education using the experience of the work in the multidisciplinary team providing both social and healthcare services. The education objective is an integral part of the project and is linked with the psychiatric care reform strategy. The outcome of the project will be the newly established integrated service connecting social and healthcare services. This service should be able to respond flexibly to a full range of needs of severe mentally ill people in the district of Cheb. Summary of project results

The aim of our project was to develop a schedule focused on deinstitutionalization and destigmatization of the SMI patients. This was realized by establishing a multidisciplinary team consisting of social rehabilitation and supported living team that is personally connected to a recently established psychiatric outpatient´s surgery for Cheb region. By connecting social rehabilitation and supported living team with recently opened psychiatric outpatient´s surgery, a new multidisciplinary team for Cheb region was created. For the duration of the project, 142 SMI patients have been provided with service and support by this multidisciplinary team. Systematic co-operation of the multidisciplinary team members with another 2 psychiatric outpatient´s surgeries enabled regular meetings, consulting and sharing common patients /clients. Thanks to introduction of the videoconference terminals (purchased within SURP project, administrated by psychiatric hospital Bohnice-Prague), we have been able to arrange a co-operation with psychiatric hospitals. Our outpatient´s surgery together with a flat for training psychiatrists were rent and equipped.14 help professionals gained working experience in the multidisciplinary team, 10 of them members of social rehabilitation and supported living team, 4 of them training psychiatrics from psychiatric hospital Bohnice. Also, special software that enables sharing patients/ clients, has been acquired. Outcomes of the project were: *Providing rent of a suitable property to run the psychiatric outpatient´s surgery according to the regulation 92/2012 *Providing rent of a suitable property to accommodate training psychiatrics *Providing technical and material equipment of the psychiatric outpatient´s surgery according to the regulation 92/2012 *Register of non-state health organization *Trial run of the multidisciplinary team and psychiatric outpatient´s surgery *Providing methodical leadership for the multidisciplinary team and psychiatric outpatient´s surgery *Creation of methodology for the multidisciplinary team and psychiatric outpatient´s surgery *Providing regular meetings of social rehabilitation and supported living team with training psychiatrics and outpatient psychiatrists plus software connection with particular care providers. *Software arrangement for sharing patients/ clients with psychiatric outpatient´s surgery connected to social rehabilitation and supported living services interconnected with co-operating outpatient´s surgeries in Cheb region.
