Description

There is a need for better coherence between health, social and other related services provided to mentally ill people. Project is focused on consecutive underprivileged community care for men with mental disorder in region Jeseníky. The aim of the project is creation of Social welfare institution and extension of services already provided by Zahrada 2000. Social welfare institution will provide the alternative to ambulatory hospital treatment and to patients discharged from psychiatric hospital will enable long - term rehabilitation and re-socialization. Within the project there will be developed psychotherapeutic programme, guided by graduate medical workers with psychotherapeutic qualification. Medical personnel of beneficiary will take lectures in seminars focused on interconnection between health and social services. The target group of the project are mentally ill people from the Jeseníky region.

Summary of project results

The aim of the project was to cover a high-quality special care of people with chronic mental disorder on Jeseniky in local underprivileged community female former Day care social welfare institution. The project had four segments: preparation of documents to register medical arrangement; education of medical workers; combination of work of field team with activity of daily social welfare institution. The goal of the project was fulfilled. The specific results of the project were: registration of a non-state health facility - a day care centre providing interdisciplinary ambulant care for people with mental illness, mostly in the form of psychotherapeutic programs; creation of a team of health and social workers prepared for the care of people with mental illness; interconnection of ambulant and field care for people with mental illness in the Jeseníky region was made – on one side methodically, on the other by adding an equipment; creation of a workplace for skills training within training facility “laundry room”. There were seven outputs of the project: registration of non-state health facility; certificates for workers from training courses; confirmation about graduation special short term attachment for workers; creation of psychotherapeutic programme for daily social welfare institution; acquisition of car; equipage of training facility – “laundry room” and making photo documentation. The target group was made of people with mental illness from the Jeseníky region. These people participated in the test station operation and work rehabilitation at the training laundry workplace. At the laundry workplace, patients were rehabilitated on a daily basis. The head of the training workplace ‘laundry’ helped people with mental illness in the workplace with the recovery of work skills. For the patients involved in the rehabilitation work was a joint support group in the morning, a full-week evaluation was held on Friday. From the 1st February 2016, a trial operation was started at the day care station. The stationary program set up a complex of individual and group activities that ran from Monday to Friday. 42 people with a mental illness participated in the testing of the day care station facility. 16 of these people were also involved in rehabilitation. 15 people with mental illness were handed over to the stationary care from a multidisciplinary field team. 2 persons from the CS were involved as members of the implementation team in the preparation of the premises for the training workplace ‘laundry’.
