Description

The project focuses on the introduction of outreach social services for the seriously mentally ill (SMI) in the region Přerovsko, Lipensko, Hranicko Kojetínsko (former district Přerov – an area of about 130 thousand inhabitants). This is a significant step in the effort to build comprehensive health and social care for the marginalized and the most needy group of patients in the region. The goal of the sub-project is an extension of existing services of opsDZ for patients who due to increased disability, absence of awareness of the disease and non-cooperation with the psychiatric outpatient care are unable to participate in the ambulatory form of the social rehabilitation. The main target group are mentally ill patients with severe diseases. The project partner will help the recipient with introduction and implementation of outreach services for the seriously mentally ill.

Summary of project results

The aim of the project was to introduce a community multidisciplinary care for seriously ill patients through cooperation of social and healthcare organizations. The ultimate goal was to create a program of comprehensive care for a group of seriously ill patients in the region. The intention was to build the Mental Health Center as a coherent and coordinated system of services with the aim to realize this program. The project goal was fulfilled. The sub-project met the sub-objectives of the program: - Deinstitutionalization of the care - Care in the patient's natural environment - Social rehabilitation and patients’ reintegration into civilian life - Prevention of other consequences of the disease - Creating a comprehensive care program The concrete results of the sub-project were: - Selection procedure for a new social worker - Creation of methodology (elaboration of methodical documents for direct work within the social service, accreditation of standards, internal regulations of the organization) - Facilities for staff background - consulting room and community team room - Purchase of a car for the mobile community service - The expert guarantor of the project developed and negotiated the conditions of cooperation with the Psychosocial Center of Přerov and Psychiatric hospital in Kroměříž in order to identify people from the target group with the need to solve their health and social situation. - Social Worker of our Public service company “Mental Health” maps the situation and needs of the clients. During the first personal contact, the client is met by a social worker and a healthcare worker. It is up to the decision of the multidisciplinary team which member of the team will contact the client next and will become his/her case manager. - Continuous mapping /evaluation of the situation and finding ways of support through the multidisciplinary team. - Contact with family, close people, client's social network. Attempting to involve the family into the solution of the situation and the fulfillment of requirements of the service user. - Implementation of concrete steps to solve the client's situation with the support of members of the multidisciplinary team. Psychiatric and social care services of the users in his/her natural environment.
