Description

There is social outreach team in Opava region, which provides care for mentally ill patients patients, but there is missing serious practical cooperation with psychiatric ambulance. The project aims to create conditions for the implementation of the transformation of psychiatric care in the region and improve the availability of comprehensive health and social services. The aim is therefore to expand and create new community services and map their usefulness. The content of the pilot project basis -create a multidisciplinary team by connection of healthcare professionals (PNO) and social (Fokus) workers. There will be prepared serve as background material for transforming mental health care in the region. It is the creation of conditions for the implementation of the transformation of psychiatric care in MSK. Project outputs – pilot traffic of multidisciplinary team, the number of completed training events and methodology pilot projects. The main target group are mentally ill patients with severe diseases. Project partner will participate on creation of the multidisciplinary team and will provide knowhow for the development of methodological documents.

Summary of project results

The aim was to realize a pilot project – basis of a multidisciplinary team which will combine healthcare workers and social workers (FOKUS – Opava, z.s.). The project also aimed at creating methodology that will serve as background material for the psychiatric care transformation in the region and thus contributing to the conditions for the implementation of the reform of psychiatric care in the Moravian-Silesian region. The specific results of the project were the following: a) Creation of the the base of multidisciplinary team - in collaboration with project partner Psychiatric Hospital in Opava (Partner without financial participation). b) Reciprocal training of healthcare and social workers – training was in these areas: "Working in multidisciplinary team in the sphere of mental health care" and conference which named "The Future of Community Care in the Czech Republic” which was attended employees of Psychiatric Hospital in Opava and employees of Fokus. c) Destigmatisation campaign - interactive workshops were organized and the project results were presented at seminars, conferences and cultural events using pamphlets or presentations d) Creation and distribution of pamphlets with the destigmatisation campaign theme and information about the implemented project. e) Preparation of methodology –multidisciplinary team methodology based on practice and experience of the team members during the project implementation was created and distributed. f) Engagement of the target group. The target group involved patients with severe mental illness - SMI - patients who need early intervention, patients at risk of spread SMI, patients with acute psychological problems and patients in crisis situations, which don’t require hospitalization.
